Field 2:	Enter the full name of each attendee who is to receive a Standard Procedures Manual. Up to 5 attendees from each bank receive complimentary manuals per month as part of the monthly pro-rata.
Field 3:	This field is optional and is to be completed only if you wish to purchase additional manuals. If you select this option, enter the full name of each attendee who is to receive an extra manual. The charge for extra manuals varies by month (depending on copying costs) and average between $35 to $50 each. Your next BCG invoice will reflect this choice.
Field 4:	This field is optional and is to be completed only if there will be additional attendees not receiving manuals. If you select this option, enter the full name of each attendee who will be attending this seminar, but will not be entitled to receive a manual. Each bank can have up to a total of 20 attendees per month.
